What to know about the Medication Therapy Management program

Taking your medications as prescribed matters — learn how the Medication Therapy Management program can help you use them safely.

Handling more than one prescription medication can be a lot to take in. That’s why the Medication Therapy Management (MTM) program is available to you at no cost — to simplify things and help you feel your best. 

If you qualify, you’re enrolled automatically and will get a letter and phone call from us. Here’s a breakdown of the MTM program: 

Who qualifies for MTM?

To participate in the MTM program in 2026, you must meet the following requirements:  

  • Have three or more long-term health conditions (like COPD, diabetes, high blood pressure, rheumatoid arthritis, Alzheimer’s disease, kidney disease or qualifying mental health conditions). 

  • Be expected to spend more than $1,276 on your medications in 2026. 

  • Take eight or more long-term medications covered by your Medicare Part D plan. 

You may also qualify if you’re an at-risk beneficiary (ARB) under a Drug Management Program (DMP). 

Remember: You can discontinue the program at any time.

6 ways the MTM program benefits you

The MTM program is here to help you gain a better understanding of your medications and feel more confident taking them. You’ll work with a pharmacist who will provide support, answer your questions and help you stay healthy. Here’s what support and services you can look forward to. 

1. Get a personalized medication review

A pharmacist will look at all the medications you take. This includes: 

  • Prescriptions

  • Over-the-counter (OTC) drugs 

  • Vitamins or supplements

They’ll explain why you take each one, how it helps your body and what side effects to watch for. You’ll have time to ask questions so you feel comfortable and informed. 

2. Talk through custom medication instructions

It’s very important you take all medications as directed by your pharmacist or primary care provider (PCP). After your review, you’ll get a list of all your medicines. It will explain detailed instructions, including: 

  • Timing — when to take 

  • Whether to take them with food 

  • Dosage

  • Other special directions 

3. Use a health plan "to-do list"

You may also get a “to-do list” from your pharmacist or health care provider. This list includes helpful tips, reminders and notes about things you may want to talk about with your PCP. It can guide you step-by-step and make it easier to follow your care plan. 

4. Talk through options and concerns

Your pharmacist will go over ways to get the best results from your medications. They can help you understand side effects, suggest safer ways to take your prescriptions and offer options if something isn’t working well for you. 

5. Find ways to save money

The pharmacist can also help you find ways to lower prescription drug costs. Some to ask about include: 

  • Choosing a generic drug instead of name brand.  

  • Checking for discounts. 

  • Looking into alternative safe and cost-effective treatment options. Their goal is to help you get the care you need without overspending. 

6. Stay supported with regular follow-ups

The MTM program doesn’t end after one call. Every few months, the pharmacist will check in with you. They’ll see how your medications are working, talk about any changes to your conditions and treatment plan, and make sure you’re still getting the support you need.
